Before your first visit, please download, print, and fill out the Patient Registration, HIPAA Consent, and Health Questionnaire. If you are the parent or guardian of a minor who will be under our care, please fill out Patient Registration for Minors, HIPAA Consent, and Health Questionnaire.
There may be one or two other forms that are required for insurance purposes. Those will be sent to you directly ahead of time or given to you to complete when you arrive for registration before your appointment time.
